CMS announces 150 participants for upcoming ACCESS model launch

The Centers for Medicare and Medicaid Services said it has accepted 150 applicants to join the Advancing Chronic Care with Effective, Scalable Solutions program, which aligns Medicare payment with measurable health outcomes.The agency's Innovation Center launched ACCESS to expand access to technology-supported care for obesity, diabetes, musculoskeletal pain and depression patients on Original Medicare.
